Imo the West will go to its usual plan of sending in weaponry, ammunition, supplies, money and mercenaries to the West's ally, this time Israel. On the other side primarily Iran will be sending in the same to its various allies in the area.
Then there is the missiles front. Israel has state of the art Western missiles, and the Iranians have state of the art Eastern missiles technology. The grind will be Israel and probably US aircraft hit targets in the area, then the Iranian proxies launch missiles into Israel.
What has changed today is that the Iranian missiles aren't 1970's era missiles that just go up in a big arc and come down unguided. Those do damage when like 200 of them are launched, but it rarely results in many dead. But precision guided missiles are a completely different ball game, they do devastating damage. And the precision missiles can be launched among a bunch of unguided missiles.
