Truecels have nothing to lose, they can fight to death. Fakecels will always hold back, want to try and survive, avoid consequences, because they have a reason to live. 
Fakecels are probably stronger.

When the truecels can use the fakecels mental instability to their advantage and attack psychologically they can get the upper hand. They have probably more gymcels as well.
