Conditions:
1. Your current appearance would stay the same, permanently. This excludes scars, temporary moles, acne, etc; the holy grail would cure all of these ailments. However, it will not realign your jaw, change your height, remove freckles, or anything like that. Essentially, anything you're genetically destined to have forever will stay, while temporary afflictions will be removed.
2. You cannot die. After all humanity on earth has left, you may voluntary die. This means that if humans leave to colonize Mars or something, and you are the only one remaining, you can still voluntarily die. No one else will ever have access to the grail. You will need to think on your decision for 24 hours before you actually die, so you won't just want to die on a whim or die on accident.
3. No one can capture you. If anyone tries to restrain you, they will die an agonizing death which will serve as a warning to all others. This includes inderict methods like burying you under rubble or throwing you in a volcano. You'll only suffer a fate like that if you let it happen. If you are stuck underwater, say you can't swim or something, you will begin to float upwards and remain at the top until you reach land after 24 hours. You will still feel the pain of not being able to breathe and all the water entering your system.

You will still feel some pain, like temperature, hunger/thirst, and blunt force, but things like bullets and sharp metallic objects will bounce off of you, and you will never get any scars or similar afflictions. You will never get sick. No government can use you as a weapon thanks to rule 3. You will still get tired and eventually you will pass out if you lose enough sleep. Your body's defense mechanisms still work while asleep.
5. Your semen never runs out.
. Your children will have your (probably) shit genes, and will not inherit your eternal life, however. If anyone tries to mess with your dick/balls it will grow as long as multiple feet and strangle them, afterwards, returning to its normal size. If a foid is giving you a blowjob and tries to bite it off your penis will extend down into their throat and clog up their airwaves.
6. After you drink out of the holy grail, you can keep it or sell it. People will likely pay billions for such an artifact, but it won't actually work for anyone else. Attempting to steal it back (if it wasn't stolen from you) gives them the same powers to restrain you, and they can move you as far away as they'd like provided it's not in an area where you'd be stuck permanently or they take longer than 24 hours taking you there.
Exceptions:
Rule 1 Exceptions: You can still get surgery, but your body will act as normal and you'll need to get a trained surgeon to do this for you. The changes are not reversible unless technology evolves to do so. If the surgeon tries to kill you or if they deliberately botch it they will die immediately and your body will automatically restore itself to how it was right before it began.
You can still gain muscle mass like a normal human via gymmaxxing, but your genes will still affect how much you gain and you will still lose muscle/gain weight over time.
Rule 3 Exceptions: If you voluntarily fight people, and they have more muscle/people than you, they may temporarily restrain you to get away. You will always escape after 24 hours, and the "restrain ability" is reset. People are aware of this condition and all other "rules" about your body, if they come into contact with you.
Rule 4 Exceptions: You can still get mentally sick, but your conditions will never worsen to the point where they make you unable to function. If you're autistic, you will still be so, and if you get/have dementia, you might forget a thing or two, but you will never wake up every day not knowing anything about yourself. Some of these ailments may pass or get better with treatment. Paralysis is counted as a physical condition.
