Serious What is wrong with my brain?

For the past two months I've been physically incapable of concentrating on anything other than my phone. I can't get myself to do any work for more than five minutes without taking an hour break. Otherwise I just blindly stare. There's like a mental block in my brain preventing me from thinking or doing anything.
